Country to Coast Home Care Services – Anglesea VIC is seeking an experienced Registered Nurse to conduct clinical assessments for older people under the Support at Home program. This casual role offers one to two days per week and focuses on assessment and review rather than ongoing nursing delivery.
You will assess in-home, identify risks, document findings, and liaise with the Clinical Care Coordinator to support protected, person-centred care.
