Key responsibilities
n
Facilitate Social Support Planned Activity Group (PAG), assist with room set up and pack away
n
Provide supervision and assistance in matters of personal care if required
n
Drive 12 seat vehicle to pick up/drop off clients attending programs and during excursions
n
Facilitate special activities, outings and events from time to time
n
Encourage appropriate participation of all clients
n
Work with individuals focusing on individual goal plans
n
Implement and observe WHS practices and procedures and report risks
n
Continue Professional Development to ensure quality of care provision
n
About you
n
Minimum Cert III+ in Aged Care/Individual Support, HACC or related qualification
n
Current First Aid Certificate (level 2) or willingness to obtain it
n
Current Working With Children Certificate or willingness to obtain it
n
Willingness to undergo current police check prior to commencing
n




Demonstrated experience driving a 12 seater vehicle
n
Proven ability in working in a team environment
n
Minimum two (2) skilled references
n
Undergo Disability Worker Exclusion Scheme (DWES)
n
Influenza vaccinations mandatory unless exempt

We are a small team of dedicated staff and volunteers who work to improve the lives of our community by providing a range of programs and activities that support each person to live independently and well.
