Nurse Practitioner
Palliative Care
to join our specialist community palliative care team. This is a unique opportunity to work within a multidisciplinary service dedicated to supporting people with life-limiting illness and their families to live well and die with dignity.
Working alongside our Medical Officer and specialist nursing team, you will play a key role in delivering advanced clinical care to patients in their homes across the Metro North region.

For over 30 years we have provided specialist community palliative care, grief and bereavement support, and carer services to individuals and families experiencing life-limiting illness.

About the Role
The Nurse Practitioner will provide advanced clinical leadership and specialist palliative care expertise, working collaboratively with the Medical Officer and nursing team to support patients with complex needs in the community.
Key responsibilities include:
Advanced Clinical Practice
Conduct comprehensive assessment and diagnosis of complex palliative care needs.
Lead management of complex symptoms, including pain, dyspnoea, delirium and nausea.
Prescribe, monitor and evaluate treatment within Nurse Practitioner scope, including anticipatory prescribing and syringe driver management.




Provide timely clinical intervention for patients with changing or deteriorating needs.
Facilitate advance care planning and goals-of-care discussions.
Clinical Leadership and Collaboration
Provide specialist consultation and clinical support to nursing staff and multidisciplinary team members.
Lead interdisciplinary care planning and complex case review.
Mentor and support capability development within the service.
Collaborate effectively with GPs, hospitals, and community providers.
Service Development and Quality Improvement
Contribute to clinical governance, quality improvement and service evaluation activities.
Support the implementation of evidence-based practice & contemporary models of care.
Participate in education, innovation and initiatives that improve access to specialist hospice and palliative care.
Contribute to the development and review of clinical resources, policies and care pathways.
About You
You are a highly experienced clinician who combines strong clinical judgement with compassion, collaboration and a commitment to person-centred care.




You are comfortable working autonomously while contributing to a team-oriented multidisciplinary team.
You bring advanced palliative care knowledge and enjoy supporting other clinicians to build confidence in complex care.
Masters of Nurse Practitioner or enrolled in same
Endorsement as a Nurse Practitioner with AHPRA
Demonstrated advanced practice experience in palliative and end-of-life care (minimum 3 years full time)
Eligibility for a Medicare Provider Number and access to relevant MBS Nurse Practitioner items
Eligibility to prescribe under the PBS
Experience prescribing and managing Schedule 8 medications
Advanced clinical assessment and decision-making capability
Highly developed communication and interpersonal skills, including facilitating sensitive end-of-life discussions
Commitment to delivering culturally safe, person-centred and inclusive care
Current unrestricted Queensland driver's licence
Experience working in community-based specialist hospice/palliative care services
Experience providing clinical education, supervision or mentorship
Experience in quality improvement, service development or clinical governance
