We have built a proud reputation for delivering holistic healthcare to our community on the Border for many years. You will be involved in a variety of family medicine including triage, health assessments, chronic disease care, preventative care and supporting patients through their healthcare journey. Working alongside GPs, nurses and a supportive multidisciplinary team, you will play an important role in care planning, referrals, procedures, wound care and health promotion

This is a role where your clinical skills meet purpose. Where care goes beyond symptoms, and you play a key part in supporting whole-of-person wellbeing, mind, body and spirit.

The Role
n
We are looking for a motivated and community-focused Practice Nurse (RN) to join our team full-time for a 12-month contract, with the potential to extend.
n
In this role you will:
n
n
Be the first point of support for clients, coordinating urgent GP appointments and managing waitlists with empathy and professionalism
n
Work collaboratively with GPs, allied health and external providers to ensure clients receive seamless, quality care
n
Provide hands-on nursing support including wound care, diagnostic procedures,



chronic disease management and health assessments
n
Assist with medication management and initial emergency presentations
n
Support health promotion and preventative programs across all ages, from children to Elders
n
n
What We Are Looking For
n
We would love to hear from you if you are:

An AHPRA Registered Nurse with a minimum of 2 years post graduate experience
n
Skilled in wound management, chronic disease care and health assessments
n
Passionate about health promotion, prevention and community wellbeing
n
Able to thrive in a fast-paced, team environment where collaboration matters
n
Strong interpersonal skills, integrity and excellent time management
n
Understanding of health issues impacting Aboriginal people
n
n
