Embedded within the Southern Adelaide Local Health Network (SALHN), you will work alongside multidisciplinary teams to provide high-quality social work services across Women and Children's inpatient and outpatient settings.

Key responsibilities

Reporting to the Discipline Lead Social Worker (Women & Children), this roles responsibilities include assessment, consultation, intervention and review, education, liaison with community agencies, organisation of appropriate resources and management of discharge.
n
Provide high quality, secure social worker care to allocated patients, engaging patients and their families or carers actively in care planning and goal setting to ensure optimal outcomes.

Ensure active participation of patients and their families/ carers in their assessment and early discharge planning.
n
Providing social worker advice and expertise to educational programs within Women's and Children's Division.

What you need
n
n
Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
n
Ability to work independently and as a member of a multi-disciplinary team.
n
Demonstrated organisational skills including the ability to adapt to fluctuating workloads.
