Transitional Aged Care Program (TACP) Physiotherapist, Prince of Wales Hospital
n
Transitional Aged Care Program (TACP) is rehabilitation in the home for older people, who have been discharged from hospital.
n
TACP is a post-acute intervention for up to 12 weeks for older people who live in the Randwick - Botany LGA.
n
There may be opportunities for additional hours based on operational requirements.
n

The Role
n
TACP has a 'restorative' purpose and aims to improve physical and social outcomes for the older people following their admission to hospital.
n
It is a goal-oriented program. Clients set their personal goals with the Clinician and work with their treating team to achieve these goals. For example, may work with someone who has experienced a Stroke and requires ongoing therapy which can be achieved in their home / community environment.

The Physiotherapist assesses mobility and prescribes a treatment plan in conjunction with the client.
n
TACP is based at the Prince of Wales Hospital Campus, Randwick. TACP is a multi-discipline team that includes Physiotherapists, Occupational Therapists, Social Workers, Speech Pathologist, Dietitian, Registered Nurses, Community Aides, Assistant in Nursing and an Administration Assistant. You will report to the TACP Manager.

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. Selection Criteria
n
To be considered for this position, please ensure you address the below questions as thoroughly as possible.

Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and the Physiotherapy Board of Australia, and demonstrated commitment to ongoing professional learning.
n
Extensive post graduate clinical experience in aged care Physiotherapy in a community setting
n
Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
n
Demonstrated ability to provide high level clinical advice, education and support to others in providing service excellence
n




Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
n
Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes
n
Demonstrated active participation in developing, implementing, and reporting on services
n
Current drivers licence (or a willingness to travel in accordance with the demands of the position
