Key responsibilities
n
Implementing participants' NDIS Plans and supporting them to achieve their identified goals
n
Supporting participants to understand their NDIS funding and maximise the value of their available supports
n
Building participant capacity to navigate the NDIS and coordinate their own supports wherever possible
n
Managing an appropriate caseload in accordance with your agreed working hours
n
Connecting participants with suitable formal, informal, community and mainstream supports
n
Maintaining strong relationships with participants, families, nominees, service providers and other stakeholders
n
Regular communication with allied health professionals, NDIA representatives and LACs
n
Conducting home visits and face-to-face participant and stakeholder meetings
n
Preparing implementation, progress and plan reassessment reports within required timeframes
n
Maintaining accurate and timely case notes and records of Support Coordination delivered
n
About you
n
Experience in NDIS Level 2 Support Coordination
n
Strong and current knowledge of the NDIS
n
Excellent stakeholder engagement and relationship-building skills
n
Strong written communication and NDIS report-writing skills
n
Excellent case management, organisation and time-management skills
n




Ability to work independently while being part of a supportive team
n
A participant-centred approach with a strong commitment to choice and control
n
Understanding of safeguarding, privacy, professional boundaries and conflicts of interest
n
Confidence communicating with participants, families, NDIA, LACs, allied health professionals and service providers
n
Relevant qualifications in Community Services, Disability, Social Work, Human Services or a related discipline will be highly regarded
n
