What you'll be doing
The Clinical Perioperative Coordinator (CPC) plays a critical role within the Perioperative Medicine Service (POMS) in the identification, coordination, and evaluation of patients waitlisted for elective surgery who would benefit from perioperative optimisation. They provide specialised coordination and clinical leadership to ensure vulnerable surgical patients are linked in with appropriate support services before surgery. This can include providing telehealth clinical advice, information on social services such as My Aged Care, and referral to prehabilitation programs.
The CPC will actively partake in the multi-disciplinary You and You Surgery (YaYS) shared-decision making clinic with Clinical Nurse Consultant, anaesthetist, intensivist, +/- member of the surgical team.



The CPC's role is to support patients in proactively optimising their health and social wellbeing before proceeding with surgery.
The CPC will be liaising regularly with the Director of Perioperative Medicine, Clinical Nurse Consultant, and Pre-Admission Clinical Anaesthetists for the holistic case management and coordination of complex patients. They will also collaborate with and complement internal and external care providers such as allied health services, NSLHD affiliates such as Royal Rehab and Hammondcare, and General Practitioners. The CPC will therefore act as a conduit, and a key communicator, between the acute and primary care settings.
The CPC will establish and maintain robust relationships with service providers, stakeholders, and organisations to sustain current pre-operative pathways. Beyond direct patient care, the CPC also provides strategic leadership as the Allied Health representative and Clinical Lead for Prehabilitation in NSLHD's Rehabilitation and Aged Health Clinical Network. This position enables ongoing promotion, development, and integration of prehabilitation into routine clinical practice across the district.
