Assignment details: Contract length: 14 weeks Shift length: 12 hour shifts Hours per week: 36 Start Date: ASAP.

Job responsibilities:

Coordinate patient care activities with physicians and other clinical team members.
n
Support safe transitions of care, including handoffs and care plan updates.
n
Work 36 hours per week with 12-hour shifts.
n
Provide patient and family education related to the plan of care and discharge needs.
n
Prepare patients for diagnostic tests or procedures and provide post-procedure monitoring as indicated.
