About the Role
n
Join CART and help people with complex health needs live well at home and stay connected to the right support.
n
You'll provide psychosocial assessment, care coordination and intervention, working closely with a multidisciplinary team to support clients and their families to navigate services and access the care they need. With an assertive outreach approach, you'll have the opportunity to visit clients in their homes across the Monash Health catchment, build meaningful connections and make a real difference to their experience of care.

You'll also contribute to service development, innovation and clinical consultation, bringing your expertise to a role where no two days are the same.

About Us

Primary Care Support Team, as part of the Community Assessment and Response Directorate offers a variety of programs that assist individuals to manage chronic health conditions,



focusing on the assessment and delivery of client focused care.
n
The team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Psychologists, Occupational Therapists, Physiotherapists, Social Workers, allied health assistants, Recreational Therapists, Leisure and Lifestyle Assistants and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health.
