About the role
n
Calvary Riverina Hospital is seeking a compassionate and experienced Social Worker to provide psychosocial assessment, support and advocacy across the hospital, with a key focus on our Palliative Care and Rehabilitation Units.
n
This permanent position offers the opportunity to work as part of a multidisciplinary team, supporting patients and their families through illness, recovery, adjustment and end-of-life care. You will play an integral role in discharge planning, care coordination and facilitating access to community supports and services.
n
This is a part-time position based in Wagga Wagga, NSW, working business hours Monday to Friday.
n
Your main duties will include:
n

Provide psychosocial assessment, counselling and support to patients and families
n
Discharge planning and coordination of referrals to community and support services
n
Collaborate with multidisciplinary teams to optimise patient care and outcomes
n
Participation in weekly MDT meetings in the rehabilitation unit
n

About you




n
n
Degree in Social Work and AASW eligibility
n
Current National Police Check or willingness to obtain
n
Experience in hospital, rehabilitation or palliative care settings
n
Strong communication and multidisciplinary teamwork skills
n
Ability to manage complex caseloads and competing priorities
n
