About The Role

As a Support Coordinator at SCS, you'll help participants understand their NDIS plans, navigate the system, connect with quality services, and advocate for their rights. It's complex work that requires solid NDIS knowledge, empathy, and a real commitment to putting people at the centre of their own lives.
SCS offers
Level 2 (General) Support Coordination
only. We do not currently offer Level 3 (Specialist) Support Coordination or psychosocial recovery coaching.
You'll manage a caseload of participants of mixed complexity. The role is hybrid - a mix of office-based and remote work - with travel for participant visits within approximately 30 km / 30 minutes of our service centre. Standard business hours with flexibility built in.
What You'll Be Doing
Implementing participant NDIS plans — understanding goals, funding categories, and how supports are best used
Helping participants and families understand NDIS plans, budgets, and the system
Identifying and connecting participants with services aligned to their plan goals
Building participants' capacity to manage their supports independently over time
Preparing comprehensive progress reports for plan reviews
Supporting participants through plan reviews and reassessments
Advocating when participants need a stronger voice in the system
Liaising with service providers, families, schools, allied health, and other stakeholders
Maintaining detailed case notes and meeting NDIS reporting requirements
Identifying gaps in local services and helping participants find alternatives
Crisis support and escalations when things go wrong
Caseload
Typical caseload is

of mixed complexity, adjusted based on case complexity and your experience.
Reporting & Supervision
Reports to: Senior Management
Clinical supervision / peer review: every 3 months
What You'll Need (Mandatory at Hire)
NDIS Worker Screening Check
First Aid certification
CPR certification
NDIS Worker Induction Modules (completed)
Working With Children Check (WWCC)
Driver's licence (current)
Personal vehicle
Full comprehensive car insurance
Right to work in Australia
Minimum 2 years' experience in support coordination, case management, or a closely related role
Comprehensive knowledge of the NDIS framework, funding categories, and Practice Standards
Excellent written and verbal communication
Strong organisational and time management skills
Nice to Have
Advanced Coordination Course (recommended)
Tertiary qualification in social work, community services, allied health, counselling, or similar
Existing relationships with service providers across the Sydney service area
Experience with plan review processes and NDIS reporting
Lived experience or personal connection to disability (family or community)
Experience supporting participants with complex needs
