About the opportunity
n
We are seeking a
dedicatedServiceCoordinator(Care
Partner)to playa central rolein coordinating
highquality,clientdirectedcare
for Bunji participants. This position ensures that every participant receives services aligned with their individual goals, preferences, and funding requirements.
n
This is a
permanent,onsiterole
that may also involve participant visits and occasional direct service delivery to support personalised care.
n
TheService Coordinatorwill:
n
n
Oversee tailored, participantcentred care across individual and group settings
n
Manage
participantonboarding,
goalsettingand care planning activities
n
Work with broader support team to align service delivery with participant goals and budget requirements
n
Coordinate supports for participants with complex care requirements, including those funded through NDIS, iCare,
NationalInjuryInsuranceSchemeand
other rehabilitation or compensable schemes
n
Support implementation of complex support plans, behaviour support plans, manual handlingplansand clinical requirements
n
Develop
andmaintainpathways
and relationships with key organisations
andindustrybodiesto
influence schemeand business outcomes
n
Assess and managerisks to participantsand the organisation
n




Support effective management of client budgets and funding arrangements
n
Manage participant experience and complaint
resolutionprocesses.
n

Who we are looking for
n
We are seeking someone who brings:
n
n
acommitment to supporting the rights, dignity, and independence of people with disabilities;
n
excellent communication and
relationshipbuilding
skills;
n
strong organisation skills and confidence inmanaging competing priorities in a fast pacedenvironment;
n
demonstrated digital competency and experience in use of digital systems and NDIS frameworks
n
strongassessment, planning, andproblem-solvingabilities;
n
leadership competencies and ability to support, influenceand coordinatethetasks of others;and
n
empathy, patience, adaptability, and a collaborative mindset.
n
n
The successful candidate will also have:
n
n
Certificate IVor higher in Disability Individual Support, Community Services or a relevant Social Services qualification
n
Extensive experience in
Disability,Community
Services, or Case Management
n
Demonstrated experience in a
similarParticipant/
ServiceCoordination
role
n
Understanding of NDIS legislation, funding structures,



and participant pathways
n
Experience supporting participants with complex conditions such asacquiredbrain injury (ABI), complex physical disability,
psychosocialdisabilityor
behaviours of concern.
n
Strong digital competency, including client record systems and scheduling tools
n
n

Hold acurrent NDIS Worker Screening
CheckandWorkingWithChildren
Check
n
Hold avalid Driver's Licence and access to a reliable, insured vehicle
n
Have had a minimumtwo doses of COVID19 vaccination and willingness to receive an annual flu vaccination

Holda valid Right to Work in Australia
