The position
As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities.
Based in Pakenham, you'll support clients across Eastern and Southern Metropolitan Melbourne, extending through to Gippsland.
Regular travel is an important part of the role, allowing you to build meaningful relationships with clients and coordinate personalised services that enhance their wellbeing, independence and quality of life.
In this role, you will:
Coordinate person-centred services within the Support at Home environment, supporting clients to achieve their goals and remain independent at home
Deliver high-quality, person-centred care and support to clients in their homes and communities
Conduct comprehensive assessments, develop individualised care plans and coordinate services
Empower clients and carers to make informed choices about their care and support
Manage your own client portfolio, including budgets, services and changing care needs
Ensure services are delivered in line with aged care quality standards and organisational policies
Maintain accurate case notes, documentation and client confidentiality
Work collaboratively within a supportive, multidisciplinary team and escalates concerns where required
Work towards agreed KPIs and service delivery targets, balancing quality client outcomes with productivity and service growth.

What's required
Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
Experience in case management, care coordination or client services within aged care, community services or a similar environment
Experience completing assessments, developing care plans and supporting clients with complex needs
Ability to manage consumer budgets and support services effectively
Sound computer skills, including Microsoft Office and client management systems
Excellent organisational skills with the ability to prioritise and work autonomously
Understanding of community aged care services, referral pathways and person-centred care principles
Knowledge of aged care legislation, quality standards and sector reforms
Current Victorian Driver's Licence and car registration
Willingness to travel across Metropolitan & Regional areas
National Police Check
