Local Community Connector Make a genuine difference in your community

We are seeking compassionate, community-focused professionals to join a national organisation supporting people with disability to access services, achieve their goals, and participate more fully in their communities.
As a Local Community Connector, you will be the local face of a nationally significant program, helping individuals and families navigate supports and services, building community connections, and working collaboratively with stakeholders to improve outcomes for participants.

About the Role
This is a rewarding, community-based position where no two days are the same. You will work directly with participants, families, carers, service providers, and community organisations to provide information, support and guidance.

Key responsibilities include:

- Providing information about available supports, services and community resources.
- Supporting participants throughout their service journey.
- Gathering and verifying information to assist with access and planning processes.
- Monitoring participant plans and assisting with implementation activities.
- Conducting participant check-ins and facilitating plan reviews where required.
- Building and maintaining robust relationships with community organisations and stakeholders.
- Promoting social inclusion, participation and community connection.
- Maintaining accurate records and completing administrative activities.

- Experience working in community services, disability, health, social services, customer service or a similar people-focused environment.
- Strong communication and relationship-building skills.




- The ability to engage effectively with people from diverse cultural and social backgrounds.
- Demonstrated empathy, resilience and professionalism when supporting vulnerable individuals.
- Sound problem-solving skills and the ability to manage competing priorities.
- Confidence working within policies, procedures and legislative frameworks.
- A commitment to delivering participant-centred outcomes.

Additional RequirementsSuccessful applicants will be required to obtain and maintain relevant clearances including:

- Working with Children/Vulnerable Persons Check
- NDIS Worker Screening Check
- National pre-employment screening requirements

The role may involve travel within local and regional communities, including visits to participant homes and community locations.
