The Role

Plan, coordinate and deliver high quality Social Work care to patients, families and carers consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.
This position sits within the Neurosciences Team and provides a social work services to patients, families, and carers across the Stroke and Spinal inpatient clinical services and the General Rehabilitation Outpatient clinical services, at Prince of Wales Hospital.
The successful applicant will deliver a high quality social work service, within their clinical areas as well as contributing to service delivery across other clinical areas in the Neurosciences Team. The role involves working collaboratively within multidisciplinary teams to support patients with complex neurological health needs across the continuum of care, to enable a safe discharge consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.
We are seeking a motivated and adaptable social worker with strong communication, organisational, and time management skills. Experience in rehabilitation, stroke, neurosciences, spinal injury or complex discharge planning is desirable but not essential. Orientation, supervision, and ongoing professional development opportunities are available.

Where you'll be working
Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria
- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
- Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
- A good knowledge of community resources and experience liaising with community organisations.
- Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
- Ability to work as part of a multi-disciplinary team and to exercise independent professional judgement on routine matters,
commensurate with years of experience
- Computer skills and the capacity to use health information systems.
- Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
Current drivers licence (or a willingness to travel in accordance with the demands of the position) and the ability to participate in the After Hours Crisis On-Call service, and work across a seven day roster (including weekends).
