About the role

Provide client-centred, evidence-based pharmacotherapy support as part of a multidisciplinary team. Collaborate with GPs to assess substance use, advise on treatment options, facilitate access to care, coordinate health services, deliver health education, and provide basic first aid to support holistic client outcomes.

- Deliver high quality alcohol and other drug (AOD) administrative services within the health centre
- Collaborate with GPs and use dedicated clinic software programs to identify clients and facilitate care planning for clients who would benefit from EPC interventions
- Support care coordination aimed at the achievement of client goals
- Undertake regular liaison with pharmacies, other AOD workers and relevant external services such as PAMs

- Tertiary qualification in a relevant discipline or working towards completion, with minimum two years relevant work experience
- Completion in or working towards, accredited training courses in pathology collection and wound management
- A level 3 First Aid certificate is highly desirable
- Demonstrated interpersonal skills, including conflict resolution and negotiation and the ability to communicate effectively with people from diverse social and cultural backgrounds

All appointments are made subject to a satisfactory Nationally Coordinated Criminal History Check (NCCHC), a Working with Children Check (WWCC), complete a Pre-Employment Declaration Form, evidence of legal rights to work in Australia, and evidence of vaccination.

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement (B). This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
