4 Hearts Support is seeking experienced and compassionate male and female Support Workers to join our team in Coolangatta to support a new participant transitioning back into the community.

This is an important opportunity to provide consistent, respectful and person-centred support during a significant period of transition. We are looking for workers who can build trust while maintaining clear professional boundaries, follow established support strategies and contribute to a safe and positive environment.

About the Role

You will provide individualised support to a participant as they transition back into community living. Support may include:

- Supporting the participant to re-establish routines and daily living skills

- Community access and participation

- Supporting appointments, activities and community-based commitments

- Assisting with household tasks and maintaining a safe living environment

- Encouraging independence, positive decision-making and social connection

- Supporting the participant to engage appropriately within the community

- Providing calm, consistent and respectful support during periods of stress or heightened emotions

- Following the participant's support plan, behaviour support strategies and risk-management procedures

- Maintaining professional boundaries at all times

- Completing accurate shift notes, incident reports and other required documentation

- Working collaboratively with the participant, family/support network and wider professional team where appropriate

About You

- Calm, patient and emotionally mature

- Reliable and consistent

- Confident working with complex support needs

- Able to maintain professional boundaries while developing positive rapport





- Comfortable working within structured routines and support plans

- Able to recognise changes in behaviour, mood or presentation and escalating concerns appropriately

- Respectful, non-judgemental and genuinely committed to supporting people to achieve positive outcomes

- Confident working independently while also contributing to a team

- Robust communicators with good written documentation skills

- Comfortable supporting community reintegration and maintaining a safe environment

Experience working with complex behaviours, forensic backgrounds, psychosocial disability, trauma, mental health, substance-use histories or other high-support environments will be highly regarded.

Essential Requirements

- NDIS Worker Screening Check

- Working with Children Check, where applicable

- Current First Aid & CPR

- NDIS Worker Orientation Module

- Relevant qualifications and/or demonstrated experience in disability or community support

- Current Australian driver's licence

- Reliable vehicle with appropriate insurance

- Ability to provide required pre-employment checks

- Availability for a range of shifts, including weekends and potentially active/sleepover support depending on participant requirements
