You will play a vital role in supporting participants to live with independence, dignity, and confidence—ensuring their voice remains at the centre of every decision. You'll work closely with individuals, families, and support teams to ensure tailored and effective service delivery. The Support Coordinator position involves working directly with participants, their families and other relevant stakeholders to ensure that the needs of the participant as defined by them are met in a timely manner.
Key responsibilities
- Building strong, collaborative relationships with participants and their networks
- Assisting participants to navigate their supports and access the right services
- Ensuring services reflect each person's goals, cultural background, and lived experiences
- Monitoring and adjusting supports to align with participants' evolving needs and preferences
- Assessment of the needs of the participant so a personalised support plan can be developed and implemented
- Linking participants with relevant services in the community and negotiating and advocating on behalf of the participant in a variety of situations
- Regularly monitoring individual plans to ensure that progress is being made towards the attainment of goals
- Communicating effectively with all stakeholders and providing guidance and information to participants and their families/caregivers to empower them to make informed decisions in regards to their support options
About you
- Effective written and verbal communication skills
- Excellent interpersonal and active listening skills
- Knowledge of disability and other services available in the community
- High levels of literacy and numeracy
- Negotiation and conflict resolution skills
- Ability to adapt communication style to meet the needs of others
- Ability to build and maintain robust community connections and relationships with other service providers, individuals and families/caregivers
- Effective time management, task coordination and administrative skills
- Competent computer skills and experience in Microsoft Office and/or specialized software such as Brevity
- Ability to work with minimal supervision
