Job Description
Mid North Coast Local Health District is seeking a Social Worker for an inpatient Rehabilitation, Stroke, and Critical Care setting in Coffs Harbour/Port Macquarie.
You will deliver patient-centred psychosocial assessments, plan interventions, advocate for patients, and coordinate discharge planning within a busy clinical setting.
You will work closely with medical, nursing and allied health teams to support positive health outcomes and family involvement.
