Job Description
West Moreton Health in Queensland is seeking a Nurse Navigator – Older Persons.
This registered nurse will translate evidence-based care into practice, coordinate complex patient journeys, and partner with families and carers to improve outcomes.
You will lead care coordination across multidisciplinary teams, apply quality improvement methods, and contribute to a patient-centered, secure, and productive health service within the West Moreton region.
