We are looking for a highly motivated, highly skilled, fun-loving person with fantastic time management and connections to allied health professionals within the Moreton Bay and Brisbane area to join our team. At Care to Change, Area Coordinators are instrumental in business development and assisting NDIS participants in the implementation and coordination of all supports within their funded plans in order for them to meet their goals.

Duties and Responsibilities:

- Work within the NDIS Code of Conduct
- Manage a portfolio of participants and assist other team members where needed
- Keep accurate and up-to-date records, with the ability to prepare reports and communication in a clear and concise manner
- Work with participants, families, decision-makers and carers to identify and access opportunities for education and employment (where appropriate) in a professional manner
- Provide proactive and effective support to participants to engage a multi-disciplinary team that meets their needs




- Establish and foster relationships with external organisations and other service providers
- File and case management with the use of cloud-based technology
- Supporting participants to understand and implement their NDIS plans
- Advocating for people to gain access to NDIS Funding
- Ability to work autonomously and as part of a team
- Engagement of recent participants in line with Care to Changes' business development
- Attend Senior Management Meetings

Essential Requirements, Qualifications and Experience:

- Minimum of 2 years experience providing NDIS Support Coordination to participants and their families
- Experience in advocating for participants to gain access to the NDIS
- Remaining up to date with NDIS systems and practices
- Understanding the importance of the NDIA and engaging with its staff, Local Area Coordinators and Planners
- Excellent time management and administrative skills to ensure that files and reports are accurate and up-to-date at all times and the ability to prioritise tasks
- Preparing for and attending plan reviews with the participant, family or representative
- Experience with business development and growing business in line with company targets
- Well-developed communication skills (verbal and oral)
- Understanding of case and file management
- Ability to report on outcomes
- NDIS Screening Card and Working with Children Card
- Hold a current driver's licence
- Right to work in Australia

Desirable Requirements and Qualifications:

- Tertiary qualification in Social Work or relevant qualification
- Experience working within an allied health sector
