Leukaemia Foundation is seeking a Supportive Care Navigator to join our Health Services team. Reporting to the Triage & Navigation Manager, you will be the first point of contact for clients and healthcare professionals, manage inbound referrals and conduct comprehensive assessments to understand individual needs across phone and digital platforms.

You will provide information, emotional support and service navigation to help people affected by blood cancer access the most appropriate supports,
