Embedded within the Southern Adelaide Local Health Network (SALHN), you will work alongside multidisciplinary teams to provide high-quality social work services across Women and Children's inpatient and outpatient settings.
Key responsibilities
- Reporting to the Discipline Lead Social Worker (Women & Children), this roles responsibilities include assessment, consultation, intervention and review, education, liaison with community agencies, organisation of appropriate resources and management of discharge.
- Provide high quality, protected social worker care to allocated patients, engaging patients and their families or carers actively in care planning and goal setting to ensure optimal outcomes.
- Ensure active participation of patients and their families/ carers in their assessment and early discharge planning.
- Providing social worker advice and expertise to educational programs within Women's and Children's Division.
What you need
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Ability to work independently and as a member of a multi-disciplinary team.
- Demonstrated organisational skills including the ability to adapt to fluctuating workloads.
