We are seeking a compassionate and experienced Registered Nurse – Care Coordinator to join our growing team. Reporting to the Clinical Services Manager, you will play a key role in coordinating and monitoring client care, combining nursing assessment, care planning, client intake, rostering, clinical coordination and staff support.

Key Responsibilities

Client Intake & Care Coordination

Manage client, family and referral enquiries and coordinate onboarding.

Conduct nursing assessments, care reviews and develop person-centred care plans.

Maintain accurate client records, assessments, consent and care documentation.

Monitor changes in client needs and escalates clinical or service risks appropriately.

Clinical Care & Support

Provide RN clinical care, assessments and interventions within scope of practice.

Support wound care, medication management, continence, chronic disease and other identified clinical needs.

Liaise with GPs, hospitals, pharmacies and allied health professionals.

Provide clinical guidance and education to support workers.

Support incident management, clinical reviews and multidisciplinary care planning.

Rostering & Service Delivery

Coordinate and optimise staff rostering and client services across multiple regions.

Match staff to clients based on qualifications, competency, availability and client preferences.

Monitor attendance, cancellations and service delivery issues.





Respond to service changes and ensure continuity of care.

CareLineLive & Documentation

Maintain accurate client, staff and service information within CareLineLive.

Monitor documentation quality, compliance and service records.

Support staff with CareLineLive training and system requirements.

Generate reports and identify opportunities to improve digital processes.

Clinical Governance, Quality & Compliance

Support compliance with Aged Care Quality Standards, NDIS Practice Standards, DVA requirements and organisational policies.

Participate in audits, quality reviews, risk assessments and continuous improvement.

Identify and escalates gaps in clinical practice, documentation, incidents and service delivery.

Contribute to clinical resources, procedures, education and regulatory readiness.

Staff & Stakeholder Support

Provide clinical guidance, coaching and support to care staff.

Identify staff training and competency needs and support induction.

Liaise professionally with clients, families, GPs, hospitals, allied health and other stakeholders.

Promote safe, person-centred, collaborative and high-quality care.

Key Selection Criteria

Minimum 3 years' nursing experience , preferably within aged care,



community nursing, disability, primary care or a related setting.

Required: Experience in care coordination, community nursing, case management and rostering or service.

Understanding of clinical documentation, incident management, escalation and care planning.

Experience using care management, rostering or client management systems such as CareLineLive or similar.

Solid organisational skills with the ability to manage competing priorities.

Ability to work independently while recognising when matters require escalation or management involvement.

Strong clinical judgement, problem-solving and escalation skills.

Qualifications & Requirements

Registered Nurse (Division 1) with current AHPRA registration – mandatory

Bachelor of Nursing or recognised equivalent qualification leading to RN registration – mandatory

National Police Check and NDIS Worker Screening Check – mandatory

Current First Aid and CPR certification

Current driver's licence and reliable vehicle

Strong computer literacy, including Microsoft Office 365 and digital clinical documentation systems
