We are seeking an experienced Clinical Care Manager - Home Care to provide clinical leadership, drive quality improvement initiatives, and support the transition to the new Support at Home program.
About the Role
- Position: Clinical Care Manager - Home Care
- Employment Type: Full-time Permanent
- Salary: $120,000 base salary + Super

Key Responsibilities
- Provide clinical leadership and oversight across the Home Care program.
- Drive clinical quality improvement initiatives and strengthen clinical governance.
- Work closely with the Home Care Manager to implement and progress an approved action plan.
- Lead Support at Home readiness activities and continuous improvement initiatives.
- Mentor and support clinical staff to deliver safe, person-centred care.
- Ensure compliance with aged care legislation, quality standards, and best practice.
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse.
- A minimum of 3 years' Home Care management experience .
- Robust understanding of the Australian Home Care and aged care sector.
- Demonstrated experience leading clinical improvement and quality initiatives.
- Excellent leadership, communication, and stakeholder engagement skills.
- A collaborative approach with a passion for delivering exceptional client outcomes.
