You will work within a multidisciplinary team to deliver specialist care to older and frail patients, enabling care closer to home and supporting early discharge from hospital into the community.

You will join the team at Ysbyty Aneurin Bevan with opportunities to contribute to a developing Older Persons pathway and to participate in multi-disciplinary
