The Clinical Care Partner will complete clinical assessments, care planning, client budgeting, reviews, care coordination and maintain their own caseload, in accordance with Support at Home, Community Home Support Programme, and other aged care specific program Guidelines. Collaborate with clients, families, carers and multidisciplinary teams to achieve positive outcomes. The Clinical Care Partner will provide clinical guidance, leadership and support to the Senior Management Team (SMT), Care Partners and Support Workers while also maintaining accurate documentation and contribute to quality improvement and clinical governance activities.

Key responsibilities

- Work within the Aged Care Quality Standards and actively work with the SMT and managers to ensure compliance with Aged Care Standards and associated legislation
- Develop, implement and monitor the clinical care and risk management of Aged Care clients
- Take responsibility for Quality outcomes in all aspects of program delivery, including participating in internal/external audits for service compliance, Responsible Incident Response Scheme (SIRS) requirements and ensuring individual practice and documentation reflects the requirements of the Commonwealth Home Support Program and Support at Home Program
- Ensure effective report writing skills and documentation in accordance with Mununjali policy




- Attend regular case management, staff meetings and training as directed
- Liaise with care workers regarding implementation of client care plans where direct care/services are included as part of their services
- Develop and implement client care plans based on assessed needs, goals and preferences to assist clients to continue living in the community
- Provide clinical support including wound care, medication management, incontinence management and catheter care
- Report and manage incidents/complaints and hazards in accordance with Mununjali Policy and Procedure
- Arrange client care, clinical care and services, products and resources from internal and external sources when required

About you

- Current AHPRA Registered Nurse registration
- Experience in aged care or community nursing
- Strong clinical assessment and care planning capability
- Understanding of the Support at Home program
- Familiar with My Aged Care and digital care management systems (eTools, TRACCS preferred)
- Current First Aid Certificate
- Current National Police Certificate
- Current C Class Driver's License, private reliable vehicle with insurance
- Demonstrate good time management skills and ability to work effectively to meet deadlines
- Utilise highly developed verbal and written communication skills
