The Nurse Navigator aims to facilitate safe and efficient care for patients who are complex by reducing service fragmentation, enhancing management in partnership with the family and carers through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey.About youOur ideal candidate will be someone who can demonstrate the following:Advanced clinical evidence-based skill and knowledge in nursing, care coordination and interdisciplinary care management,
