The purpose of this role is to provide exceptional, safe, evidence-based, and patient centred nursing care to the patients of Platinum Health Group. The practice nurse delivers a high standard of professional care that supports positive patient outcomes and aligns with best practice clinical standards.

The Practice Nurse works collaboratively within the practice team and across other PHG sites, as well as liaising with allied health professionals and health care providers in the greater community. Demonstrating initiative and accountability, the nurse contributes to the effective and efficient operation of the practice while maintaining professional standards and working within their scope of practice and relevant AHPRA requirements.

Key responsibilities

- Deliver high-quality, evidence-based nursing care across acute, preventative and chronic disease management.
- Coordinate patient care and advocate for positive health outcomes through collaboration with GPs, allied health and external healthcare providers.
- Proactively identify opportunities to improve patient health through preventative care, recalls, screening and chronic disease management initiatives.
- Work collaboratively with the Nurse Supervisor to maintain high clinical standards, patient safety and quality care outcomes.




- Contribute to continuous quality improvement activities and support practice accreditation requirements.
- Maintain professional competence through ongoing learning and development.

About you

- Minimum 1 year's post registration experience
- Current RN AHPRA registration and professional indemnity insurance
- Strong communication and time management skills
- A caring and collaborative approach to patient care
- Commitment to ongoing career development
- Ability to work independently and demonstrate initiative — identifying and addressing practice needs proactively
- Basic computer literacy and telephone skills
- Previous experience working in general practice or community-based nursing (desirable)
- Competence in the use of relevant equipment and software (desirable)
- Additional qualifications such as Accredited Women's Health Certificate, Diabetes Education Certificate, Asthma/Spirometry training, advanced wound caret certificate, Immunisation Nurse Qualification/recognised immunisation training (desirable)
