Key Responsibilities

The Qualified Support Worker will:

- Read and understand consumer and participant care plans and deliver services in accordance with the documented care requirements.
- Provide personal care and daily living assistance in a respectful, caring and professional manner.
- Recognise when a consumer or participant’s needs,



abilities or condition have changed from what is documented in their care plan and promptly report this to the Care Manager or Coordinator.
- Escalate concerns regarding changes in a consumer or participant’s health, wellbeing, behaviour, safety or general condition.
- Maintain the privacy, dignity and confidentiality of consumers and participants at all times.
- Complete all required documentation, including progress and shift notes, accurately and within the required timeframe.
- Provide domestic assistance and maintain agreed areas of the consumer or participant’s home to a high standard, including bathrooms, kitchens, living areas and other designated areas.
- Assist consumers and participants with shopping while following their individual preferences, instructions and available budget.
- Safely transport consumers and participants to appointments, social activities, shopping and community outings while supporting his comfort, dignity and wellbeing.
- Prepare meals in accordance with individual dietary requirements, preferences and documented instructions.
- Encourage independence and support consumers and participants to remain actively involved in decisions relating to their care and daily activities.
- Follow all organisational policies, procedures, infection-control requirements, work health and safety requirements and protected work practices.
- Report incidents, hazards, near misses and concerns promptly in accordance with organisational procedures.
- Work collaboratively with Care Partners, Coordinators,



clinical staff and other members of the care team to ensure continuity and quality of care.

About You

To be considered for this role, applicants must have:

- Successful completion of Certificate III in Individual Support (Ageing and/or Disability), Certificate III in Aged Care , or another recognised qualification suitable for providing home care services.
- A satisfactory professional reference from a current or recent employer demonstrating competence, reliability and confidence in performing support work.
- A National Police Check that meets current organisational and legislative requirements.
- A current First Aid Certificate .
- Current CPR certification .
- Current Manual Handling training .
- A current Working with Children Check (Blue Card) where required for the role.
- A current NDIS Worker Screening Clearance (Yellow Card) where required for the role.
- Evidence of the right to work in Australia .
- A current unrestricted or appropriate Australian Driver Licence .
- Access to a reliable, registered and appropriately insured vehicle for work-related travel.
