Clinical Engagement and Partnerships Coordinator

This is a part time, permanent position, Monday, Wednesday and Friday, 9am to 5pm, based at our Concord office with travel across Sydney. You'll own the front door — building and holding the relationships that bring participants to us, and making sure every referral that arrives is handled properly and quickly.

Key responsibilities

- Build and maintain referral relationships with hospitals, discharge planners, clinical teams, allied health and community organisations
- Manage referrals as they come in — triage, respond, gather what's needed and hand over cleanly to the service team
- Represent Aus Care Community Services at meetings, network events and with referring professionals
- Track where referrals come from and what happens to them, and report on it
- Work with the Managing Director on which relationships and areas to prioritise
- Make sure referral conversations reflect participant choice and our conflict of interest obligations

About you

- A background in disability, health, mental health or community services, with credibility in front of clinical professionals
- Experience in referral management, intake, partnerships or business development in a health or disability setting
- Solid working knowledge of the NDIS,



particularly how participants come into the scheme and what happens at discharge
- Confidence starting conversations with people you don't know yet
- Organised follow-up — the relationships are built in the second and third contact, not the first
- Current NDIS Worker Screening Check, Working with Children Check, driver's licence and comprehensively insured vehicle
