Premier Aged Care facility located in Sydney Eastern Suburbs is seeking a Clinical Care Coordinator

As a Clinical Care Coordinator, you will assist the Director of Care in the development and progression of high standards of clinical care delivery.

n

n

n

Responsibilities Include:

n

n

n
- Oversee and lead the team to ensure excellent service delivery and that nursing practices and procedures are consistent with legislation and policies

n
- Coordinate communications and services between residents, families and medical professionals

n
- Directing and maintaining infection control

n
- Occupational health and safety standards

n
- Assisting in commonwealth government funding process

n
- ​ Ensuring that education programs and quality improvement initiatives are implemented

n
- Comply with the Accreditation Standards and Guidelines for Residential Care Services

n

n
Requirements n

n

n




- Current Nurses Registration with AHPRA

n
- A solid background in Aged Care at an RN level

n
- Minimum of 3 years’ experience in a similar role

n
- Demonstrated knowledge of aged care accreditation standards, resident appraisal, classification processes and ANACC

n
- Proven experience leading, motivating and managing a team

n
- Excellent IT skills with the ability to implement and evaluate the use of IT systems in a residential aged care environment

n
- A strong passion for quality in aged car

n
- Excellent communication & interpersonal skills

n
