About the role

n

We are seeking a reliable and compassionate Support Worker to provide high-quality, person-centered support to NDIS and Aged Care clients in the Ulladullah, St Georges Basin to Nowra Shoalhaven Heads areas. We are seeking a support worker who will build strong rapport and bring consistency. You'll provide disability and social support; helping them live with dignity, independence, and joy.

Key responsibilities

n

n
- Provide high quality and timely care support to the client as directed by Home Caring, and in accordance with their care plan
n
- Safely transport the client using your own vehicle
n
- Support client in getting out for social engagement, activities and in attending appointments
n
- Provide personal care and daily living (domestic) assistance
n
- Assistance with household tasks (may include cleaning, meal preparation)
n
- Document service activities that support service outcomes
n




- Support participant in achieving individual NDIS goals
n
- Maintain accurate documentation (including reporting) and shift notes
n

n

About you

n

Qualified experience in the NDIS and/or Aged Care sectors. Minimum 1 year experience and Cert 3 as a minimum requirement

n

Excellent communication skills, commitment to a person-centred focus and flexibility to support needs based on individual circumstances

n

Demonstrated experience providing compassionate, person-centred support to individuals living with dementia

n

NDIS Worker Screening Check

n

Working with Children Check

n

Current First Aid certificate and current CPR certificate

n

Current Drivers Licence and access to a reliable vehicle with full comprehensive insurance

n

The right to work in Australia

n

Availability for minimum of 12 hours per week
