Key Responsibility and Duties

- Conduct thorough in-home assessments to identify clients' physical, emotional, and social needs, then design individualized support, training, and educational programs.
n
- Interview clients and their families to evaluate the nature and extent of difficulties and collaboratively develop goal-oriented care plans.
n
- Regularly monitor client progress against care plans, documenting outcomes, and adjusting interventions as required to promote independence.
n




- Refer clients to appropriate health, welfare, housing, or employment agencies to access additional services and resources.
n
- Liaise with community groups, government bodies, and private providers to raise awareness of available resources and address local service gaps.
n
- Support families by providing education, respite, and guidance for caring for children or adults with disabilities in home and group settings.
n
- Assist young people and vulnerable adults to resolve social, emotional, or financial challenges through practical advice and advocacy.
n
- Prepare funding submissions and detailed reports for government and funding bodies to secure resources for client programs.

Requirements

- 1 year of full time work experience within community services, aged care, or disability.
n
- Relevant qualifications are required.
n
- Strong communication, documentation, and interpersonal skills.
n
- Valid driver's license and willingness to travel to client homes.
