About the Role:

n

We are seeking an experienced and compassionate Clinical Care Coordinator to join an established and accredited NFP residential aged care provider in Melbourne. This is an excellent opportunity for a clinically strong Registered Nurse who enjoys leading teams, supporting high standards of care, and making a meaningful difference in the lives of residents.

n

Working closely with the Clinical Nurse Manager, you will support the day-to-day delivery of clinical care services, provide leadership to nursing and care staff, and help ensure residents receive high-quality, person-centred care.

Key Responsibilities:

n

- Coordinate and oversee clinical care across the facility
n
- Support comprehensive care planning, assessments, reviews, and documentation
n
- Provide clinical leadership, guidance, and mentoring to nursing and care staff
n
- Ensure care is delivered in line with residents' individual needs, preferences, and goals
n
- Monitor clinical indicators, incidents, risks, and continuous improvement activities
n




- Support compliance with the Aged Care Quality Standards and organisational policies
n
- Liaise with residents, families, GPs, allied health professionals, and external stakeholders
n
- Promote a positive, resident-focused culture across the facility
n

About You:

n

To be successful in this role, you will bring:

n

n
- Current AHPRA registration as a Registered Nurse
n
- Previous experience in residential aged care, ideally in a senior clinical or care coordination role
n
- Strong knowledge of care planning, clinical documentation, assessments, and aged care compliance
n
- Excellent communication, leadership, and problem-solving skills
n
- The ability to support, motivate, and develop clinical and care teams
n
- Full working rights in Australia
n
- Current National Police Clearance, COVID and flu vaccination
n
