An Aboriginal Shire Council is seeking a Community Care Coordinator to help Elders and community members access respectful, reliable support that enables them to live well and independently.

n

Based in remote Cape York, this is a relationship-focused role requiring cultural awareness, sound coordination skills and a genuine commitment to community-led service delivery.

n

You will work closely with clients, families, care workers, service providers and community stakeholders to keep services responsive and well organised.

About the Role

n

You will coordinate culturally safe community care services and support the consistent delivery of practical, person-centred assistance.

n

n
- Coordinate services for Elders, people with disability and other eligible community members
n
- Work with clients and families to understand needs, goals and service preferences
n
- Assist with care plans, service reviews, referrals and follow-up actions
n
- Coordinate care workers, schedules and day-to-day service priorities
n
- Build effective relationships with health providers,



agencies and community organisations
n
- Monitor service quality and respond promptly to gaps, concerns and changing needs
n
- Maintain accurate client records, service information and program documentation
n
- Support reporting, compliance and continuous improvement activities
n
- Promote culturally safe, respectful and person-centred service delivery
n

About You

n

You will combine practical coordination ability with patience, cultural sensitivity and a strong commitment to community wellbeing.

n

n
- Demonstrated experience in community care, aged care, disability support, health services or a related field
n
- Ability to build culturally safe and respectful relationships with Aboriginal and Torres Strait Islander peoples
n
- Understanding of person-centred care, client choice and community-based service delivery
n
- Experience coordinating services, workers, referrals or client support activities
n
- Solid communication skills with clients, families and service partners
n
- Ability to maintain accurate, confidential records and meet reporting requirements
n
- Calm judgement and the ability to manage competing priorities
n
- Capacity to work independently and adapt to the realities of a remote community
n
- A relevant community services, aged care, disability or health qualification will be highly regarded
