Job Description

Cafs is seeking a Home Based Care Case Coordinator in Ballarat, VIC. This full-time permanent role focuses on coordinating care placements for children and young people in foster and kinship arrangements.

You will work with carers, Child Protection, families, schools and health professionals to support case plan goals and placement stability. The role emphasizes trauma-informed practice, relationship-building, advocacy and collaboration to secure secure, nurturing homes and meaningful family
