We triage vists that are close to home to reduce your travel. This is a hybrid role mixing time between home visits, working from home and time in the office.

About the Role

n

This is a full-time permanent position, with flexibility for part time applicants.

n

As a Community Occupational Therapist, you will deliver high-quality, person-centred care through home visits across Maitland and the upper Hunter. We offer a highly flexible caseload that can be tailored to your clinical interests, experience, and preferred style of practice.

n

Whether your passion lies in NDIS/disability care or aged care, we will structure your caseload to suit you:

- NDIS / Disability Focus: Deliver Functional Capacity Assessments (FCAs), SIL & SDA assessments, assistive technology prescription, home modifications, and therapy aimed at building independence and capacity.
n
- Aged Care Focus: Support older adults under the Support at Home program to remain living independently, through assessment, intervention, and preventative care.
n

We aim to provide a diverse but clinically meaningful caseload, ensuring you are both challenged and supported in your role.

Key Responsibilities

- Manage a community-based caseload with structured triage to ensure it aligns with your scope of practice
n
- Conduct comprehensive assessments in clients' homes and community settings
n
- Prescribe assistive technology and complete home modification recommendations
n




- Deliver therapy and capacity-building interventions in the home or clinic environment
n
- Complete high-quality clinical documentation, reports, and stakeholder communication
n
- Liaise with families, support coordinators, and other health professionals to achieve optimal outcomes
n
