Pathway Talent is partnering with a leading not-for-profit organisation to recruit two SST Intake & Assessment Officers to support the delivery of the Supporting and Sustaining Tenancies (SST) Program.

About the Role

n

As an SST Intake & Assessment Officer, you will coordinate intake, assessment, referral, and service allocation processes to ensure participants receive the right support at the right time.

n

You will work closely with participants, community partners, support services, and internal teams to identify needs, assess risks, facilitate referrals, and create pathways to improved wellbeing and housing stability.

n

This role is ideal for someone who combines strong assessment and administrative capabilities with genuine compassion and a commitment to person-centred practice.

Key Responsibilities

n

- Conduct intake, eligibility, risk, and comprehensive assessments.
n
- Review referrals and determine eligibility, urgency, and prioritisation.
n
- Coordinate participant onboarding and service allocation.
n
- Facilitate referrals to appropriate internal and external supports.
n
- Maintain accurate records, databases, case notes, and reporting requirements.
n
- Build positive relationships with participants, community services, and key stakeholders.
n
- Promote recovery-oriented and strengths-based approaches to service delivery.
n
- Contribute to positive housing and wellbeing outcomes for participants.
n

About You

n

To be successful,



you will be passionate about supporting individuals experiencing mental health challenges, housing instability, or social disadvantage.

n

n
- Experience in mental health, homelessness, housing, community services, intake and assessment, service coordination, or case management.
n
- Knowledge of recovery-oriented and person-centred practice.
n
- Strong communication, engagement, and relationship-building skills.
n
- Excellent organisational, administrative, and time-management abilities.
n
- The ability to manage competing priorities while maintaining attention to detail.
n
- A collaborative approach and commitment to achieving positive participant outcomes.
n
- Qualifications in Community Services, Mental Health, Social Work, Psychology, Human Services, or a related discipline will be highly regarded.
