About the opportunity

We are seeking a dedicated Service Coordinator (Care Partner) to play a central role in coordinating high quality, client directed care for Bunji participants. This position ensures that every participant receives services aligned with their individual goals, preferences, and funding requirements.

This is a permanent, onsite role that may also involve participant visits and occasional direct service delivery to support personalised care.

The Service Coordinator will:

- Oversee tailored, participant centred care across individual and group settings
- Manage participant onboarding, goal setting and care planning activities
- Work with broader support team to align service delivery with participant goals and budget requirements
- Coordinate supports for participants with complex care requirements, including those funded through NDIS, iCare, National Injury Insurance Scheme and other rehabilitation or compensable schemes
- Support implementation of complex support plans, behaviour support plans, manual handling plans and clinical requirements
- Develop and maintain pathways and relationships with key organisations and industry bodies to influence scheme and business outcomes
- Assess and manage risks to participants and the organisation




- Support effective management of client budgets and funding arrangements
- Manage participant experience and complaint resolution processes.

Who we are looking for

We are seeking someone who brings:

- a commitment to supporting the rights, dignity, and independence of people with disabilities;
- excellent communication and relationship building skills;
- strong organisation skills and confidence in managing competing priorities in a fast paced environment;
- demonstrated digital competency and experience in use of digital systems and NDIS frameworks
- robust assessment, planning, and problem-solving abilities;
- leadership competencies and ability to support, influence and coordinate the tasks of others; and
- empathy, patience, adaptability, and a collaborative mindset.

The successful candidate will also have:

- Certificate IV or higher in Disability Individual Support, Community Services or a relevant Social Services qualification
- Extensive experience in Disability, Community Services, or Case Management
- Demonstrated experience in a similar Participant / Service Coordination role
- Understanding of NDIS legislation, funding structures, and participant pathways
- Experience supporting participants with complex conditions such as acquired brain injury (ABI),



complex physical disability, psychosocial disability or behaviours of concern.
- Strong digital competency, including client record systems and scheduling tools

All Bunji employees are also required to meet the following compliance:

- Hold a current NDIS Worker Screening Check and Working With Children Check
- Hold a valid Driver’s Licence and access to a reliable, insured vehicle
- Have had a minimum two doses of COVID 19 vaccination and willingness to receive an annual flu vaccination
- Hold a valid Right to Work in Australia
