About the role

As a Care Partner, you will support Paramedical Healths customers throughout their care journey, helping them achieve their goals and remain living safely and independently at home or in a place of their choosing. Working closely with customers, families, clinical teams and external providers, you'll develop holistic care plans, coordinate services and ensure customers receive person-centred support. This is a Permanent Full time opportunity, Monday to Friday, supporting SAH customers.

Key responsibilities

- Build trusted relationships with customers and support them to achieve their goals at home.

- Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.

- Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.

- Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.

- Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.

- Recognise and escalate customer risks,



safeguarding concerns and changing care needs through appropriate pathways.

- Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.

- Promote safety, quality and person-centred care across all aspects of service delivery.

About you

- Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.

- Strong customer focus, with the ability to build trusted relationships and navigate sensitive conversations with empathy and professionalism.

- Excellent communication skills and confidence using Microsoft Office and customer management systems.

- Knowledge of funding streams, quality standards, audit requirements and care transitions, including discharge planning.

- Strong decision-making, problem-solving, time management, and prioritisation skills in a fast-paced setting.

- A collaborative mindset, with the ability to work effectively across multidisciplinary teams, families, clinicians and external providers.

- Relevant qualifications in Community Services, Aged Care or a related discipline (Certificate IV or Diploma desirable).
