Centacare is seeking experienced Clinical Assessor Delegates to join our Single Assessment Services (SAS) team, delivering high-quality aged care assessments, triage and delegated decision-making across Queensland.

About the Role
We are seeking a Clinical Assessor Delegate to deliver end-to-end aged care assessments within the My Aged Care framework, combining clinical expertise with strong operational performance.

Reporting to the Clinical Assessor Team Lead, the Clinical Assessor Delegate is responsible for delivering aged care assessments, triage and delegated decision-making functions within the My Aged Care framework. The role undertakes Comprehensive Assessments, Home Support Assessments and Support Plan Reviews (SPRs) using the Integrated Assessment Tool (IAT), determines eligibility and support needs, and exercises approved Assessor Delegate authority to finalise service approvals and funding decisions in line with legislative and contractual requirements.

This is a dynamic, community-based role that requires a high level of autonomy, initiative and accountability in managing your caseload, travel, scheduling, documentation and competing priorities. You will be responsible for organising and prioritising your workload and making day-to-day decisions within the scope of your role, under the direction and guidance of the Team Lead.



You will work collaboratively with the Team Lead and broader team to ensure alignment with service priorities, respond effectively to changing needs, and achieve contractual service timeframes and key performance targets.

Key Responsibilities

- Deliver high-quality, person-centred, culturally safe and trauma-informed aged care assessments across community and remote settings, incorporating wellness and reablement principles.
- Undertake Comprehensive Assessments, Home Support Assessments and Support Plan Reviews using the Integrated Assessment Tool (IAT).
- Review, triage and progress referrals within contractually required KPI timeframes.
- Maintain an average productivity benchmark of two completed assessments per working day, or other service benchmark as directed.
- • Exercise approved Assessor Delegate authority to finalise service approvals and funding decisions in accordance with legislative, contractual and program requirements.
- Maintain documentation accuracy and 100% compliance with My Aged Care delegation authority, legislative requirements and program guidelines.
- Contribute to audit, quality improvement and governance activities.

What You Bring

- Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy or Social Work, with current AHPRA registration where applicable.
- Minimum one year experience delivering aged care assessments or working in geriatric services.
- Completion of, or capacity to successfully complete,



required My Aged Care Workforce training and Assessor Delegate training requirements.
- Demonstrated ability to independently manage a caseload and achieve quality and productivity outcomes in a KPI-driven environment.
- Strong clinical reasoning, documentation and decision-making capability.
- Confident communication and interpersonal skills, with the ability to engage older people, families and multidisciplinary stakeholders.
- Solid digital capability and confidence working across multiple systems and Microsoft Office.
- Current driver’s licence and ability to obtain and maintain required compliance checks, including NDIS Worker Screening, Australian Federal Police criminal history check, Blue Card and First Aid/CPR certification.
