Right at Home Newcastle Toronto provides in-home care to older people and is seeking a Care Partner to join our team on a full time basis. You will onboard recent clients, assess needs, build care plans, and stay with them as their situation changes, mixing office work with visits to clients at home.
You will work with families, GPs and the care team, keeping records up to date, handling enquiries, and mentoring care workers while ensuring services match each client’s plan.
