Country To Coast Home Care Services – Anglesea VIC

Country to Coast Home Care Services is seeking an experienced Registered Nurse to conduct clinical assessments for older people receiving services under the Support at Home program. This is a flexible, casual position working one to two days per week. The role focuses on clinical assessment and review rather than ongoing care planning or regular nursing service delivery.

Key responsibilities
Conduct comprehensive clinical assessments in participants' homes
Identify clinical risks, health concerns and changing care needs
Complete clear and accurate clinical documentation
Make appropriate recommendations and referrals
Communicate assessment findings to the Clinical Care Coordinator and relevant team members
Support safe, person-centred and evidence-based service delivery
Escalate urgent clinical concerns appropriately





About you
Current registration as a Registered Nurse with AHPRA
Relevant clinical nursing experience
Solid assessment, communication and documentation skills
Understanding of clinical risk and safeguarding older people
Current National Police Check
Current driver's licence and reliable vehicle
Ability to work independently in a community-based setting
Experience in district nursing, community nursing, aged care or a similar assessment-based role would be highly regarded
