Grampians Health at Home seeks a Fractional Specialist Physician or General Practitioner to deliver geriatric and complex care across the Grampians region, including hospital and community settings.
You will provide clinical leadership, participate in education, and support a multidisciplinary team in a senior role within Hospital Without Walls.
You will lead telehealth assessments, coordinate care, and contribute to policy and quality improvement while advancing regional healthcare partnerships
