Workforce Extensions Sunshine Health is seeking a compassionate and reliable Care Partner for our client, Solara Health.
If you have experience supporting older people in their homes and are passionate about providing high-quality, person-centered support, we'd love to hear from you.
What do you need?
Qualifications or experience in Nursing is preferred
Experience in Case Management is preferred
Strong time management and organisational skills
Excellent verbal and written communication
Ability to build positive relationships with clients, families, and advocates
Ability to work autonomously and in a team environment
Current Police Check, driver's licence
Duties & Resposibilities
Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement




Work with clients to identify strengths and abilities and deliver appropriate care and services
Develop, review, and update care plans to support client goals
Liaise with medical, allied health, and community stakeholders
Maintain regular client contact, including monthly monitoring calls
Implement and manage client budgets
Ensure compliance with Aged Care Quality Standards
Speaking a second language is an advantage
Vehicle provided/own: company vehicle provided
Pay rates: $80K + super
Reporting to: Community Care Manager
