You will work with multidisciplinary teams to achieve optimal outcomes for clients, from Aged Care funding through activities like developing and reviewing care plans and managing budgets.
You will spend much of your time on the road, traveling between Bowen, Proserpine and surrounding areas, building solid relationships with clients and the broader care team.
