Job Description
Right at Home is seeking a Registered Nurse to join the Clinical Care Partner team, delivering autonomous, client-centred in-home care.
You will lead clinical care, coordinate services, and supervise care staff while guiding clients to live safely and with dignity in their own homes.
You will manage your caseload, perform clinical assessments, and develop care plans aligned with client goals and funding.
Travel to clients' homes is expected, with robust clinical leadership support.
