Registered Nurse – Community / Home-Based Care

We are seeking experienced and patient-focused Registered Nurses to deliver in-home education and sub-cutaneous therapy support for patients prescribed specialist medications.
n
This is a flexible, community-based chance suited to nurses who enjoy autonomy, patient education, and working one-on-one in the home environment.
n
About the Role
n
In this role, you will support patients and/or their carers through:
n
n
Education and training on self-administered sub-cutaneous injections
n
In-home sub-cutaneous infusions, including pre- and post-observations
n
Documentation, reporting, and adverse event monitoring in line with program requirements
n
n
Visit Expectations
n
n
Patient contact to be made within 48 hours of referral to arrange a mutually convenient visit
n
Visit duration:n
n
Injection training: approx. 1.5 hours
n
Infusion visits: approx. 2.5 hours
n
n
Completion of visit documentation and time recording within 24 hours via a secure online portal
n
Prompt reporting of any adverse events, incidents, or spills in accordance with protocols
n
n
Training Provided

Comprehensive online training program covering:n

Essential Requirements
n
n
Current Registered Nurse registration (AHPRA)
n
Current CPR certification
n
Demonstrated experience with Sub-cutaneous injections AND IV cannulation
n
High attention to detail and strong clinical documentation skills
n
Excellent communication and interpersonal skills
n
High level of computer literacy and confidence using online portals
n
Flexible availability (days and times)
n
Access to a safe, roadworthy vehicle
n
Current unrestricted Australian driver's licence
n
